Faculty Video Request Form

To schedule a videotape or DVD for in-classroom viewing, complete this form at least 10 days prior to viewing. The item will be checked out to you and available for pickup 24 hours prior to the viewing date.

If the video is not available in the Library's collection, we will request a short-term loan through interlibrary loan or investigate purchasing the video.
